A completed entry form must have your first and last name, e-mail address, mailing address, phone number and age (you must have a valid email address to participate in this sweepstakes). Entries must be received between April 19, 2008 and 11:59 pm (Eastern Standard Time) on May 7, 2008. Finalists notified by phone must respond within seven (5) calendar days or an alternate finalist will be chosen.
